IMPORTANT INFORMATION FOR PATIENTS

 

 

What Happened?

 

On February 15, 2011, we learned that an external hard drive containing personal information of some patients of MidState Medical Center had been misplaced. The information contained on the device consisted of names, addresses, dates of birth, marital status, Social Security numbers and medical record numbers. We promptly began an investigation and subsequently reported the event to law enforcement authorities. The individual responsible is no longer employed by our business associate, Hartford Hospital, or any other Hartford HealthCare affiliate. We have no reason to believe that any personal information has been misused as a result of this incident.

 

 

 

What Information May Have Been Compromised?

 

The information contained on the device consisted of names, addresses, dates of birth, marital status, Social Security numbers and medical record numbers of some patients of MidState Medical Center. Not all of the patients being notified of the incident had Social Security numbers on the missing hard drive. We have no reason to believe that any personal information has been misused as a result of this incident.

 

 

 

What efforts have been made to find the misplaced hard drive?

 

After we learned that an external hard drive containing personal information of some patients of MidState Medical Center had been misplaced, we promptly began an investigation and subsequently reported the event to law enforcement authorities. We also retained a private investigator to search for the hard drive, but it has not been found. MidState Medical Center and other affiliates of Hartford HealthCare are in the process of reviewing their policies and are taking steps to help ensure that this type of incident does not happen in the future.

 

 

 

What are you going to do to help potentially affected individuals?

 

We have sent a notification letter to affected individuals. We provided those individuals with information they can use to help protect themselves. We have arranged to offer affected individuals with two years of identity protection under the Debix Identity Protection Network at no charge to them. In addition, we have provided affected individuals with information on obtaining free credit reports and placing fraud alerts on their credit files.

 

 

 

What are you going to do to help potentially affected individuals?

 

I am/was a customer of MidState Medical Center, but I didn't get a letter. Was my information affected? This incident involves a defined group of customers of MidState Medical Center. Individuals who may have been affected by this incident are being notified by mail.
